I am having trouble finding resources to help me with a paper on Cognitively Disabled Adolescents with Chronic Illnesses Transitioning to Adult health Care
I am also needing information regarding cognitively disabled patients and their response to change.

There are a number of databases in Ebscohost that can provide you with the information you are looking for. Try combining a search in Ebscohost, by searching the following databases together: Cinahl, PsycArticles, Psychology and Behavioral Sciences Collection, Medline, Health Source Nursing. So when you enter Ebscohost, first check the boxes next to each of them before you do the search and then click continue. Make sure that before you do your search you go below the search box to the limiters ad limit your search to scholarly peer reviewed articles and full-text articles. You can do that by checking boxes for those in the limiter section.

In the search box try various keyword term combinations until you are able to find what you are looking for. When you put in your terms, be guided by the keyword system, because as you put in your terms, at the bottom of the search box it will give you terms that might apply. Those terms are usually set or guided vocabulary terms and Ebsco indexing terms, so try to use them. 

I tried the terms chronic disease patients  AND cognitive disorders and found 508 full-text articles. I also tried chronic disease patients AND cognitive disorders AND adolescents and found only 15 full-text articles. I tried chronic disease patients AND cognitive disorders AND transitions and found 4 full-text articles.  On your second question I tried cognitive disorders AND reaction to change   and found 139 full-text articles.You might try to replicate my searches.
